Years ago, a wrestling fan wanted to test if Inoki was as tough as he was made out to be in wrestling, so he punched Inoki. Inoki, understandably pissed, slapped the fan REALLY hard. The fan actually thanked him, and the incident became well known in Japan. Ever since the, people have asked him to slap them as a weird way to install courage and test their mettle.
Inoki is basically the Hulk Hogan of Japan, probably the most famous pro wrestler ever. The slap is one of his signature moves. Basically, getting your picture taken with Inoki slapping you is like getting a picture of the Hulkster dropping a leg bomb on you or Superfly Jimmy Snukka coming off the top rope on you. It's pretty cool, actually.
